JOB DESCRIPTION: IOP Counselor/Recovery Support/Family:
Facilitate patient groups. Develop an individualized treatment plan, according to ASAM criteria within 2 weeks of admission, and embellish throughout the course of treatment. Enter into computer. Chart all 1x1's. Chart results of all family contacts and 1x1's. Update treatment plan with medical and psych staffing recommendations. Chart other recommendations and action planned, approved or taken; for example, arrangements for patient appointments, appointments with attorneys, outside medical personnel, etc. Attend and participate in assigned staff meetings. Complete a thorough discharge/transfer plan according to OAR's and company policy. Familiarity with and follow Serenity Lane fire and safety protocols and procedures. Familiarity with and follow Serenity Lane personnel and program policies and procedures. Conduct self with integrity, honesty, respect and patience around staff and patients.
